Is my address printed on the Basic Disclosure Certificate?

Your Name and Date of Birth is printed on your Basic Disclosure certificate.

The address is provided on the certificate is our address:  C/O: Mayflower Disclosure Services Ltd, 76 Rollesbrook Gardens, Southampton, SO15 5WB.
 
Your address is NOT printed on the certificate.  Please ensure this meets your needs, before ordering a Basic Disclosure Check through us.

What Information will I need to provide on my Basic Disclosure Application?

To complete your online Basic Disclosure Application, you will need to provide: note: Information that must be supplied is indicated with an asterisk(*).

  • * Name/s (including  previous name/s and dates you used them)
  • * Birth details; i.e.: date of birth, town/country of birth; note: at least one copy of ID documents that you provide for your ID verification must show your date of birth
  • * Full 5 year address history (incl. dates); note: at least one copy of ID document that you provide for your ID verification must show your full address of where you currently live
  • * COPIES of the required ID documents - see list of Acceptable ID documents
  • Passport details - if you have a Passport
  • Driving Licence details - if you have a Driving Licence
  • Email address (for correspondence)
  • Credit/Debit card or PayPal details to arrange payment
Incorrectly filled-in applications, queries, or incorrect ID will cause delays and may result in the rejection of your application.
In such cases of rejection, or late cancellation, administration charges may be applicable. 

Please note: if you are obtaining a Basic Disclosure Check for someone else you must obtain their express permision (in writing), to do so. 

How do you verify my ID?

All applicants must be able to provide COPIES of their ID documents so that we can verify your identity.  These can be sent to us by emailing scanned copies to us or by posting photo-copies to us in the post (tracked recorded delivery recommended).  Full instructions will be provide to you when you Start your Online Application.

Applicants must provide copies of at least 3 x ID documents. Please see the list of acceptable ID documents.   


Important note:  Please check this list carefully to ensure you can provide copies of the correct ID documents. Time constraints also exist for some documents.  These are indicated on the Acceptable ID list.

At least one ID document must show the full address of where you currently live, and at least one ID documents must show your date of birth.

 
If you cannot provide copies of ID in line with the Acceptable ID list we will not be able to process a Basic Disclosure application for you.

 

At least one document must be:

  1. Valid passport (any nationality)
  2. UK Driving Licence Full or Provisional – England/Wales/Scotland/Northern Ireland/Isle of Man; either photo card or old-style (pre-1998) paper driving licence)
  3. Original UK birth certificate (issued within 12 months of date of birth, full or short form acceptable)
  4. Valid photo identity card (EU countries only)
  5. UK Firearms licence
  6. HM Forces ID card (UK)
  7. Adoption Certificate (UK)
You are then required to provide copies of at least two other documents as detailed in the List of Acceptable ID.

NB:  If you do NOT have one of these ID documents we CANNOT process a Basic Disclosure Application for you.

How Long do the checks take?

On average it takes 13 - 16 working days (Monday - Friday not including bank or public holidays) to process a Basic Disclosure Certificate from the receipt of a correctly filled in application form (note any errors on the application form or queries could cause delays in the processing).   As a guide, 95% of applications are completed in 12 calendar days. 

This is measured from the day we receive your online application and copies of ID as detailed in the Acceptable ID list ; to the day of dispatch. Basic Disclosure Certificates are dispatched by standard Royal Mail postage.  Please note:  seasonal fluctuations/delays will occur during certain times of the year for example during Christmas and Easter.


Our sole business is the processing of criminal records checks.  We will do everything we can to ensure your Basic Disclosure is processed as quickly as possible (see our customer testimonials).

What will I receive?

One hard-copy of the Basic Disclosure Certificate is produced and we post that to your current home address (as provided on your application form), by Royal Mail Standard delivery (for UK applications) and by Royal Mail Airmail delivery (for non-UK applications). 


Who will receive my Basic Disclosure Certificate?


Your hard-copy Basic Disclosure certificate will be posted to your current home address, which you provide on your online Basic Disclosure Application form.  Please note, this address cannot be changed once your Basic Disclosure Application has been submitted.  If you move, it will be your responsibility to arrange for your mail to be re-directed to your new address.

Certificates for UK residents are dispatched by Standard Royal Mail.

Certificates for Ex-UK residents living Overseas are dispatched by Royal Mail Airmail.  If you live in a country that has an unreliable postal service,we can dispatch the Basic Disclosure Certificate to you by International Courier.

Please note, your Basic Disclosure Certificate will not confirm your current address.  It will state C/O: Mayflower Disclosure Services as the registered address.

How/When do I pay for my online application?

You will be prompted for payment when you reach the end of your online application.  If you do not complete your payment at this time, your application will be cancelled and your form details will be lost.  Payment can be made by debit / credit card PayPal.

The system is asking me to enter an Organisation Reference and Password - what are these?

To access the secure online Basic Disclosure Application form you need to enter an Organisation Reference and Password.  These are provided in your instructions before you Start an Online Application.


Apostille Service

An 'Apostille' is the legal term for the legalisation of a certificate, official proof that it is what it claims to be.


We do NOT provide an Apostille Service.  Please arrange this yourself once you have received your Basic Disclosure certificate.  For advice about getting your certificate Apostilled, contact the Foreign and Commonwealth Office (FCO).
