Basic Disclosure Application Pack

For UK residents To purchase one Basic Disclosure application pack please click on the BUY NOW button to the right. Once your online request and payment has been received we wiil send you by post a Basic Disclosure (Police Check) application pack containing an application form, and instructions. Complete the application form and return it to us at Mayflower Disclosure Services with photo-copies of: 1.        Passport, Driving License or Birth Certificate plus 2.        Utility bill that confirms current home address plus 3.        One more ID document from Acceptable ID list Mayflower will check and arrange for the processing of your application. On receipt of your Basic Disclosure Certificate we will post this back to you. How long does it take? On average it takes 12 - 16 days to receive a Basic Disclosure Certificate from the receipt of a correctly filled in application form (note any errors on the application form could cause delays in the processing (95% are completed in 12 days). Who will receive the Certificate? One copy of the Basic Disclosure Certificate is produced and we forward that on to the person who requested it. How long is my application pack valid for? If you do not use your application pack the fee will not be refunded.  Please submit your application within three months of receiving it to ensure your application form is still valid and the price has not changed.
