Once your online request and payment has been received, we will post to you via Air Mail a Basic Disclosure Application Pack with instructions, to any postal address in the world.

Complete the application form and return it to us at Mayflower Disclosure Services with photo-copies of: 1. Passport, Driving License or Birth Certificate plus
2. Utility bill that confirms current home address plus
3. One more ID document from Acceptable ID list

On receipt of your completed application form Mayflower will check and arrange for processing of your form.

Your Basic Disclosure Certificate will be posted back to you via Royal Mail International signed for Air Mail.
How long does it take?
From receipt of your correctly completed Basic Disclosure Application to you receiving the Basic Disclosure Certificate is approx 3 weeks. The time can vary dependent on which country it is returned to and other factors such as public holidays, local postal delays. (Note, any errors on the application form, incorrect addresses etc. could cause a considerable delay in its processing)
How long is my application pack valid for?
If you do not use your application pack there is no refund. Please submit your application within three months of receiving it to ensure your application form is still valid and the price has not changed. VAT is included in the price. In the event that VAT or the Disclosure costs increase, we may pass this cost on to you at our dIscretion.
